I created this form to use with ABeka Academy Lessons. When the lesson for each day is completed, in the COMPLETE column, I write the date it was completed. This is one of the ways that I keep records for our home school. It could be used with other curriculums too, but I created it specifically for use with Abeka Academy lesson days.

If you live in NC and would like to home school your children you will need to complete a notice of intent to home school, which can be found online at the North Carolina Department of Non Public Education. This website will also help answer your questions about home schooling in North Carolina.
Each year I refer back to the North Carolina Department of Non Public Education's website to update my enrollment information and print out a registration record. Some places offer discounts for home school families so I like to keep a copy of the registration record on me. I also use the NC DNPE guidebook below for reference and on the last page of the guide book you will find an attendance record form you can print out. This is the form I use to keep daily attendance for our home school.
